    In seeking to participate in “StandUP” 2022 (Leader of the Pack)

    Residential Address, _____________________________________________ Do hereby warrant and acknowledge;

    1. That my general health is good and there is nothing which renders me unfit to undertake this event.

    2. That I understand and appreciate fully the fact that there may well be risks, hazards, and dangers involved to which I would be subjected, more particularly:

    a. We will be partaking in intense physical exercise.

    b. That I agree to discontinue engaging in any activity immediately upon the feeling of any unease, disoriented, or experiencing any lack of well-being or due to any medical reasons that arise during my participation. Without derogating from the foregoing, I agree to abide by the instructions given by Leader of the Pack personnel to discontinue activities should they cite my safety as an immediate concern.

    3. That I agree that by my participation in the “StandUP” 2022 (Leader of the Pack), I hereby assume all risks, dangers and all responsibility for any losses and/or damages, and/or injuries whether caused in whole or in part by the negligence or other conduct of the owners, staff, agents, or by any other person, to the extent permitted by applicable law.

    4. That I understand and agree to follow all health and safety rules, regulations & procedures and in the event that such rules and procedures are not followed by myself, or others, it could

    result in serious injury or death to myself or others and/or damage to and loss of use of property. I understand that such rules, regulations and procedures (or any additions and amendments made thereto from time to time) may be conveyed to me in a variety of means, including verbally or in writing (including by electronic means) by the Leader of the Pack staff.

    I accept the 'standard conditions of contract' and voluntarily assume the risk inherent in taking part in such an event and I, together with my heirs, executors and administrators hereby release Leader of the Pack its officers, servants, agents, coaches and representatives, from any duty or care towards me, in connection with my participation in this event, and from liability from all or any claims that could accrue to me or my heirs, executors and administrators arising out of my participation in this event and its activities or in any related activities irrespective of whether such claim or claims arose through the negligence of any person, or from any of the risks, dangers or hazards inherent in this event, or of any loss of, or damage to, any property from any cause whatsoever and I further indemnify and hold harmless associated persons against any claims howsoever the same may arise, to the extent permitted by applicable law. To the extent permitted by applicable law, I hereby waive the protections of any applicable law whose purpose, substance and/or effect is to provide that a general release shall not extend to claims, material or otherwise, which the person giving the release does not know or suspect to exist at the time of executing said release. If any provision of this Release shall be determined by a court of competent jurisdiction to be invalid or unenforceable to any extent, the remainder of this Release shall not be affected thereby except as may be necessary to make the remaining provisions consistent with each other after the invalid or unenforceable provisions are deleted, and each provision hereof shall be valid and shall be enforced to the fullest extent permitted by law.

    This Agreement shall have worldwide effect but the laws of the State of Israel shall govern the rights and obligations of the parties to this Release and the interpretations, construction and enforceability thereof.

    Leader of the Pack reserves the right to use any photograph, video, audio recording or any other media taken of me during “StandUP” 2022 (Leader of the Pack) or in connection with any other activities, in its promotional materials, brochures, web-site, and any other advertising. I have carefully read this agreement and fully understand its contents. I am aware that this is a release of liability. I understand that this release is a legal contract and that I sign it of my own free will, in clear judgment and in good faith, free of any duress or undue influence, having had the opportunity to receive independent legal or other advice regarding its terms. I agree to be bound by its terms. I further understand that this agreement has no expiration date.
